Ozzy Osbourne’s cause of death has been revealed two weeks after he passed.

The Black Sabbath frontman died from a heart attack, according to a death certificate filed by his daughter Aimée Osbourne at a London registry and obtained by the New York Times Tuesday.

The certificate lists “out of hospital cardiac arrest,” as well as “coronary artery disease and Parkinson’s disease with autonomic dysfunction” as causes.

The former reality TV star died on July 22 at the age of 76, according to a family statement obtained by Page Six.

“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” the statement read.

“He was with his family and surrounded by love. We ask everyone to respect our family privacy at this time.”

Weeks before his death, the iconic rocker performed at a Black Sabbath farewell concert.

The heavy metal band took to Instagram July 22 to pay tribute to the pioneering musician. Alongside a photo of Osbourne performing, they wrote, “Ozzy Forever.”

The rock legend left behind his wife, Sharon Osbourne, and children Aimée, Kelly and Jack.

He was also father to Jessica and Louis Osbourne, whom he shared with ex-wife Thelma Riley, and her son Elliot Kingsley, whom he adopted.

The singer had battled Parkinson’s disease in recent years, as well as the debilitating effects of injuries stemming from a 2003 quad biking accident.

Days prior to Ozzy’s death, his daughter Kelly addressed a fake AI video claiming her father was dying.

“It has a voice like my dad’s David Attenborough or something. And it starts out saying, ‘I don’t need a doctor to tell me that I’m going to die. I know I’m going to die,’” the reality TV star said on her Instagram Stories.

“What the f–k is wrong with you people? Why would you spend your time making a video like this?” Kelly said, insisting that Ozzy was “not dying.”

“Yes, he has Parkinson’s, and yes, his mobility is completely different than it used to be, but he’s not dying,” she said.

The “Fashion Police” alum also addressed the alleged suicide pact between her parents after Sharon’s past comments on the matter.

“Stop making articles or posts about how you think my parents are having a suicide pact,” she said. “That was bulls–t my mom said to get attention one time. And my dad’s not dying. Stop.”

Sharon discussed the alleged pact on an October 2023 episode of “The Osbournes Podcast,” divulging that she and Ozzy had made a plan to go to Switzerland and use the services of physician-assisted suicide organization Dignitas.

At the time, the “Talk” alum said she preferred not to “suffer,” adding, “I don’t want it to actually hurt.”

“Mental suffering is enough pain without physical,” she said. “So if you’ve got mental and physical, see ya.”
